Legal Shifts & Dangers of Compounded Tripeptides (GLP-1s): Why the Landscape Has Changed

In recent years, compounding pharmacies have filled a demand gap by producing off-label “copycat” formulations of GLP-1 / peptide medications such as tirzepatide and semaglutide. These compounded versions often touted lower cost or customized dosing to patients when branded supplies were limited. However, the FDA has now officially declared that the shortage of tirzepatide injections is resolved, which triggers key legal restrictions on compounding that drug.

Because tirzepatide is no longer considered in shortage, compounding the drug without a very narrow, patient-specific medical justification is generally illegal. These regulatory changes have serious implications. Many compounding pharmacies have already ceased producing tripeptide/GLP-1 compounds. Some providers and telehealth platforms that continue to advertise compounded tirzepatide may be operating in legal risk, particularly since the FDA has also issued warnings about fraudulent compounded GLP-1 drugs that mislabel or misbrand their contents.

Besides legal risk, compounded versions carry safety concerns:

  • Because they aren’t subject to the same rigorous manufacturing, testing, and stability requirements as FDA-approved medications, there’s greater risk of contamination, incorrect dosage, or degradation.

  • The FDA has received reports of adverse events linked to compounded tirzepatide — including injection site reactions and dosing errors.

  • Without regulatory oversight, patients may not know the true contents or purity of what they’re injecting.

Why At Blue Iris We Prescribe Brand-Name Zepbound (Tirzepatide)

Given these changes, at Blue Iris Medical Aesthetics we no longer rely on compounded tripeptide or compounded tirzepatide options. Instead, we prescribe brand-name Zepbound (tirzepatide) — the FDA-approved formulation — offering:

  • Legal compliance and safety: We adhere to updated FDA and compounding regulations, ensuring patients are not exposed to illegal or unsupervised compounded products.

  • Quality assurance: Zepbound is manufactured under strict standards, with validated dosages, stability, and oversight.

  • Transparent pricing: We offer Zepbound at very competitive pricing, even for clients without insurance. No insurance required.

  • Consistency in dosing and results: Because it’s the approved brand, you receive the correct formulation and dosing, reducing variability and risk.

We believe that your safety, transparency, and long-term success should never be compromised for cost — especially when regulatory shifts leave compounded alternatives legally and ethically untenable.
